All requests and inquiries concerning The Plan's medical policies that are related to medical devices and technologies must be directed to:

Please do not contact individual Plan medical directors or medical policy clinicians.
When submitting data on a new or existing medical technology, please send an email to Michele Galvan with the information below:
- A subject line that states the type of technology (existing or new) and the specific technology name
- A detailed description of the request and desired outcome. Please be as specific as possible and include copies of relevant, published, peer-reviewed studies.
At this time, meetings are not routinely scheduled. If a meeting is required to further discuss the submitted evidence, The Plan will contact the vendor representative.
